What is REACH?

  • You know yourself or the person you care for best.
  • Tell us if you have a serious concern about your condition or notice a worrying change.
  • The REACH program helps you to raise your concerns with us.

How does REACH work?

  • You may RECOGNISE a worrying change or have a serious concern about your condition. Or you may recognise the worrying change in the person you care for.
  • If you are worried, ENGAGE with those caring for you. Speak to the nurse, midwife, doctor, or allied health professional. Tell them your concerns.
  • If they do not help with your concerns or the condition is getting worse, then ACT. Ask to speak to the person in charge of the area.

      Your call will be answered by a staff member at Grampians Health.
  • HELP is on the way. A health professional from Grampians Health will act on your concerns within 30 minutes.
